Prior to arriving at camp on Sunday, please make sure all forms are complete and up-to-date in your UltraCamp account. Pay special attention to your camper's health form. Please ensure you have answered the COVID vaccination question in your campers UltraCamp account. Even though he or she may have come to camp the summer before, please go in and update it regarding medications or other conditions that may have changed in the past year. In addition, all allergy information should be on your camper's health form, including food allergies.
Items to have ready at check-in:
- Completed camper 7 day pre screen form
- Camper OTC and/or prescribed medication(s) (if applicable)
- Camper medication form signed by physician (if applicable)
In order to complete the check in process on Sunday, you will need to bring any over the counter or prescription medications for your camper with you. Please make sure you have the medication form complete and signed by a physician. Medications should be in their original prescription bottles with the correct dosing on the container. Over the counter medications should also be in their original containers. Please send just enough medication for the time your camper is to be at camp.
